What’s the diagnosis? Answer and explanation below!!!

40 year old female was referred to me for a left inguinal hernia. She has had a bothersome mass in the left lower quadrant, on the lateral aspect of her Pfannenstiel incision, for the past two years. The pain is worse with menstrual cycles. It gets larger and painful during her menses, and smaller but still present after 8 days from the start of her cycle. On exam, she has a firm, immobile mass that is not typical of an inguinal or incisional hernia. What does she have?

Very frequently, bulges, bumps, masses, etc. of the abdominal wall are diagnosed as hernias by non-surgeons. (See my previous diastasis recti posts). Her history and exam clearly point to endometrial implants, or endometriosis. Endometrial implants occur when the lining of the uterus get displaced and grow where it shouldn’t be. In this patient, this likely occurred from her Caesarian section (I mentioned her Pfannenstiel incision). During her c-section, where the uterus is cut open to deliver her baby, part of the lining of the uterus fell off and seeded into her abdominal wall. The CT shows two implants. The first one outlined is within the left rectus abdominus muscle, and the second one (the one that was palpable on exam), is within the subcutaneous layer). The endometrial implants got larger and painful during her menstrual cycle because they are still viable tissue, and therefore still respond to the hormonal changes that occur during a period.

This amazing video shows a pretty typical ENT reconstructive surgical technique called a Lefort 1 osteotomy. It is commonly used to fix a wide range of midface deformities. As you can see, with the mobility achieved by the Lefort 1 osteostomy, you can correct defects that require advancement, retrusion, elongation, or shortening. This patient has multiple facial fractures. The sinuses and nasal passages are crushed. The air he breathes is obstructed coming out the nose so it escapes under the skin, this is called subcutaneous emphysema (air within the soft tissues). Air escaping the fractures sinuses is causing this ballooning affect of the soft tissues. We see this often in motor vehicle accidents, unprotected falls, assaults, etc. Treatment is surgical repair of the facial fractures and not decompression of the air within the soft tissues. The air will resorb on its own and only antibiotics are needed after surgery to repair the fractures.

Answers to the CT quiz from yesterday. This is a 60 year old male who came to see me for a large ventral, peri-umbilical hernia. His pre-op work up revealed a platelet count of 90k, which is low. He denied any known liver issues such as alcoholic cirrhosis, hepatitis, etc. I ordered a CT scan which I posted. Here are the findings:

Photo 1: Straight on view of his large hernia. 
Photo 2: Side view of his hernia. His belly button is gone
Photo 3: CT slice showing the obvious hernia with fat protruding through it. This is located at the top of the screen, in the middle. A hernia is a hole or defect in the abdominal wall, which you can easily see here. You can also see the hernia sac outline. 
Photo 4: Inside the hernia is a large collateral varix, or an enlarged vein. This is because...
Photo 5: the patient has an undisclosed history of pancreatitis. He said it was never figured out why he had it. The usual causes of pancreatitis are alcohol abuse, gallstones, certain medications, or autoimmune. He denies any of that. His pancreatitis caused fluid to accumulate around the pancreas, forming a pseudocyst (the arrow is pointing to this). The splenic vein runs next to the pancreas and when there is fluid or a mass, it can compress, or obstruct, the splenic vein. When there is obstruction of the splenic vein, the blood flow gets diverted to the smaller blood vessels, which get larger. This CT shows huge, dilated veins everywhere in the abdomen. So in summary, his pancreatitis caused a pseudocyst to form, which caused blockage of his splenic vejn, which led to engorgement of the other smaller veins, and also caused his spleen to sequester, or eat up his platelets, causing his low platelet count.
